Ability Case Management

Ability Case Management coordinates person-centred rehabilitation and support for individuals with Acquired Brain Injury, Spinal Cord Injury and/or Orthopaedic Injury. We are committed to listening to our clients and their family members to provide a comprehensive assessment and intervention plan tailored to individual needs. Working in collaboration with approved service providers we proactively lead the rehabilitation and support process in partnership with clients and their family members. We are resilient and persistent, breaking down barriers in our determination to achieve the best possible outcomes for our clients. Ability Case Management is a highly organised, diligent and resourceful service with the goal of helping individuals reach their full potential following injury.
