MedZed is a nationwide company with local field teams that help members with social determinants of health. In California we specialize in Enhanced Care Management and Community Support Services. We locate and engage your members and reintegrate them into primary, specialty and behavioral health care and dental services to close critical care gaps and keep them out of the emergency room and the hospital. MedZed brings hands-on care management to members with uncontrolled spend, directly in their communities and homes.

Our locally-based Navigators have lived experience of many of the same challenges—and how to overcome them. They are deeply passionate about helping others in need.

We take them from crisis to care. Our impact includes engagement of 40% of assigned members, 70% PCP appointment completion rate, and a 40% reduction in ED visits per thousand.
