Pathways Community HUB Institute is a Toledo, Ohio-based nonprofit dedicated to partnering with communities to advance whole person, community-based care through the PCHI® Model – an outcome-oriented framework that empowers local networks to improve health for all.

Founded in 2015 by Dr. Sarah Redding and her late husband, Dr. Mark Redding, the organization initially developed the Pathways Model in 2001, inspired by their work with community health workers in Alaska and Baltimore.

The PCHI® Model provides training and tools for CHWs to identify risk factors and work towards eliminating those risks one by one, linking payment directly to outcomes. It’s a quality improvement framework for communities to build their own robust network of community-based care coordination in partnership with local stakeholders to align resources and achieve positive outcomes.

Today, the PCHI Model is implemented in 40 communities across 17 states, demonstrating a proven approach to improving population health and reducing costs.
