We are here to put general practice at the heart of joined up care in Salford We have outstanding health and social care services in Salford but local people have told us they would like to see people involved in their care working more closely together. All the city’s GP practices have agreed to work together in five neighbourhoods – Broughton; Eccles and Irlam; Little Hulton and Walkden; Ordsall and Claremont and Swinton. We are responsible for driving this collaboration forward and supporting the practices to pool their services, staff and resources so that all GP practices in the neighbourhood are strengthened and improved. This also means connecting general practice with other professionals and support in local neighbourhoods, such as district nurses, social workers, mental health workers, voluntary organisations and local community groups. This means people will have better access to the advice, expertise and support they need to be as independent as they can, with the opportunity to see a wider range of health professionals and access the full range of activities in their area. Local people will require fewer hospital admissions and visits as there will be more skills and support available within the community they live
